The Magothy River Index is an assessment of the health of the tidal river for communication to the residents of the watershed. It measures the percentage attainment of three health goals in the river: water clarity (Secci disk visible at 1 meter), dissolved oxygen (5 mg/l at all depths), and submerged aquatic vegetation (SAV) (100% cover as visible by airplane). It has been produced annually since 2002.

In 2015, The Magothy River Index (Download Report) was 33% (a D grade), a slight decline from 2014, due to a decrease in dissolved oxygen concentrations and water clarity.
